birth certificate

how do i go about having my name added to my daughters birth certificate? as i am the biological father. her mother will acknowledge this fact?

As the biological father, you should contact the bureau of vital statistics for a form called "acknowledgement of paternity." Fill out and file the form as instructed. If you would like me to fill out and mail the form to you for your signature, send a check for $50 with your request to my address, along with the names and social security numbers of you, the mother, and child.
