if a divorced spouse moves away years after the divorce who is responsible for paying the cost of transportation for child visitation

Unless the decree provides otherwise, the "visitation parent" provides all transportation.

Was there any geographic restriction in the last order?

Costs of exercising visitation CAN be a factor in modifying child support.
