am I liable if knew about a safety issue and didn't report it and someone got hurt?

You could be, it depends upon the facts. Are you a property owner? Did you create the safety issue? The question is whether you had a duty to act based upon the knowledge. Duty is typically determined by your relationship to the injured party and/or control of the mechanism of injury. This question cannot be answered without knowing the facts.
