Verbiage in final judgement of PI

Following mediation, I accepted a settlement and have the following questions:

1) The verbiage of the mediation agreement states that I will be responsible for tax on the settlement amount. Is this correct?

2) The final judgement verbiage states that a judgement will be issued against me, the plaintiff, as a result of the case being settled before reaching court. Is this correct?

3) I am being asked to secure a Medicare/Medicaid lien release, even though neither agency has placed a lien on the case. Is this correct?
