Legal Question in Credit and Debt Law in Utah

I have been granted a judgement for a case against my ex husband on an amount of money he owes to me and I have been trying to collect it and I have decided to do a garnishment and I have started to fill out the garnishement papers and I think I have done it wrong... do I fill it out against him or the employer where he works at?

A garnishment is filed with the case heading and served on the party holding the money you are trying to get. Go to the court's website for instructions.
