You should go to the clerk's office of the particular court which handled your matter and inform the clerk that you would like to petition that court for a restricted driver's license privilege. The clerk will take it from there and may ask you to complete some papers regarding the request.

You file a petition in the Circuit Court where you live. You send a copy to the Commonwealth's Attorney. At the first hearing if the CA doesn't have any problems with the request (like some other legal reason you shouldn't drive) the judge will refer you to DMV's ASAP program - you will be evaluated by DMV as to what their requirements would be (driving school, drug or alcohol treatment, etc) The judge will set a return date for ninety days and after the DMV report is received you will be allowed to have a restricted license while you fulfill the requirements. Once the requirements have been fulfilled, you will return to court to see if the temporary license can be made permanent.

It's easier to do with an attorney.
