What are the requirements for a landlord to give a tenant a 5 day notice?

I think what you refer to is a 5 day pay or quit letter. This is a notice sent to a tenant who has failed to pay the rent and, if he fails to pay the rent within 5 day, he has to quit, or move, from the premises.
