correct legal agreement

not sure what form to use ?

involved agreement that payment to be made for medical errors conditions lead to one party hospital expenses and lost of work days. Agreement would need to insure there will be no future legal liability after the payment.

It sounds like a release. A release is where on party (the wrongdoer) agrees to pay the victim a certain amount of money in exhange for an agreement not to make any future claims--regardless of whether the injury was fully appreciated when the settlement was made. Unless the victim is a minor these are generally enforceable--meaning that if you accept the money and sign the release you cannot bring any future claim; so be careful and make sure that you understand fully the loss and long term effect of the injury.
