Legal Question in Personal Injury in Virginia

In August of 2010 I was rear-ended through no fault of my own. I have received some treatments and tests (MRIs, XRays, etc), but almost a year later I am still in a moderate-to-high level of pain. The insurance company of the woman who rear-ended me has sent me a letter to settle and has followed up with a phone call. I have not yet responded, since I am not sure what to do- I am still in a lot of pain, and there are other treatments to be tried; I may have a chronic pain issue and agreeing to the settlement with free the woman's insurance company from any responsibility. How should I proceed?
