Who can (and how do they) file a motion requesting the disbursement of surplus funds from a non-judicial trustee's sale? Please include information on downloading any and all free versions of any and all forms required. (Lewis County, WA)

Its not free and there are no forms. Any attorney in Lewis county who understands real property can help you.

Or, you can contact the Lewis County Bar Association and ask for a referral or if they have a clinic.
