Prison sentence modification or reductions

I am looking for any and all information on prison sentence modifications or reductions. I would like to know if there is a standerdized form for such action or any specific formate or guide lines that must be followed. I was thinking the court could be possibly approached over this matter through A WRIT of some type ? I don't know which one would apply here though. Any help would be appreciated thank you for your time-

There is a modification package floating through the legislature that may be part of an overall reform package for truth-in-sentencing but for the moment the only way to effect such a reduction is to demonstrate that there is a "new factor" present that was not known at the time of sentencing. This does NOT include rehabilitation efforts or any kind of treatment. It is a very difficult burden to meet.
