Signing full custody rights to mother before baby is born

I am pregnant - due late in December. The biological father is moving away, and has agreed to sign over all rights to me. We would like to know if there is any paperwork or forms we can sign to make this legal before the baby is born. If not, what can we do to make this all official as soon as possible?

We have a form that he would need to sign: "Consent to Termination of Parental Rights". He could sign it at any time.

I would be interested to know whether he also expects that you would be waiving your right to child support? If so, that is a different form that we also have.
