How Do I File A Claim in Arizona?

I was recently hurt on a job that was covered by Workmans Comp. I have seen two doctors who both agreed there is nothing they can do to correct the injury, which has resulted in a 25% loss of the use of my right arm.

Can you please tell me the process and type of claim I may file (in Arizona) for compensation? Any information on what my options are and how to proceed would be greatly appreciated.

You indicated that your injury was covered by workers' compensation, but then you go on to ask how to file a claim, so I'm confused about whether you're referring to a claim for your injury, or a claim for permanent disability.

You can call the Industrial Commission Claims Department (they're in the phone book--LawGuru doesn't let me include phone numbers in my answers!) to assure that you do have a claim filed. If you don't, it is critical to get it filed ASAP. You technically have one year to file a workers' compensation claim, but waiting that long will cause you problems with the additional requirement that an injury be "reported forthwith".

If you are only talking about permanent disability, you don't have to do anything. The industrial carrier will issue a notice indicating how much permanent impairment you have suffered, and declaring their position about how much they think you should get for permanent disability (if anything).
