I'm trying to file a writ of mandate they say i need other forms to attach what would they be?

Depends on what kind of case. Normally you would prepare a memorandum of points and authorities, and you would attach exhibits consisting of all the court's minute orders and transcripts of all hearings that you want the appellate court to consider, and a proof of service.
