Re: subpoena form #

Perhaps a SUBP-010, but there are only five or six Judicial Council forms covering the whole field of subpoena, so you should look at them all. A non-lawyer cannot issue a subpoena - you will need the help of your lawyer or the court clerk - and either of them can advise you on the correctness of the for you select. If you are in Small Claims - probably Form SC-107.

Re: subpoena form #

It depends on whether you are in small claims, limited jurisdiction or unlimited jurisdiction, family court, etc.

You can find a list of available forms at http://www.courtinfo.ca.gov/cgi-bin/forms.cgi

Try choosing to look at all forms by number, and then search within the page for ones that have "subpoena" in the title
