Why are you attempting to fill out court forms without at least consulting a lawyer? Isn't winning the case important to you?

You cannot serve the papers yourself. You need someone over the age of 18 who will serve the papers and sign the proof of service by mail. This is the simplest past of the case and you are having problems with it. Therefore, that is a strong indication that you need help from a professional. You should see a lawyer but if you definitely cannot afford one at least get a paralegal so that you can fill out the right forms.
