How common is it for a minute order to be incorrect?

I see it happen every now and then. The minute order is prepared by the clerk, and is not reviewed by the judge. I have even seen some judge's start yelling at the clerk for messing up the minute orders. If it is serious, you need the court reporter's transcript, which rules when it conflicts with a minute order.
