need to know what form is needed for supervised visitation

Was told i needed a form to have a monitor with me when i visit my child

In most cases you do not want to have supervised visitation. In my experience once the court orders supervised visitation it is difficult to stop it and the cost can be extremely high.

You can without an a court order take an individual with you on a visit.

If you are requesting that visitations with the other parent be supervised, you will have to convince the court that the parent presents some kind of danger to the child or children. The court may order supervised visitation if there is sufficient evidence of risk to the children.
