Guardianship of minor children

I want to legally appoint a guardian for my children,(if something happens to me). The children live in California and the guardian that I have chosen lives in Oregon.

Can this be done while I am still alive?

Are there legal forms that can be notorized without hiring an attorney?

Are there parental rights you are not disclosing. If another parent, he/she may have rights upon your death. Otherwise, you may be able to.
