Legal name after divorce

When I completed my divorce papers I checked the box stating I wanted to go back to my maiden name. My divorce papers came in my married name. Is my legal name now my maiden name? How do I find out for sure?

The judgment should state that you former name is restored. If that box is not checked and your former name is not stated on the court order, your former name was not restored. You should file a motion to correct the final judgment to restore your former name, as it was left off as the result of a clerical error.
