Legal Question in Insurance Law in California

According to HIPPA regulations and guidelines, is there a legal document that must be signed to allow others to inquire on anothers policies?

Does each company usually generate a form according to their needs and as long as the general info is on that form it can be acceptable?

Another person needs a Hippa Authorization. Many companies have their own forms, but they all generally contain the same wording. Get a copy from the insurer you are dealing with, as that is the easiest way to do it. If not, ask them what formt they require, if any.
