Losing a home to Medical

I recently purchased my Grandmothers home from her, but yet she still lives in the home with me. If she goes onto Medical and ends up in an Assisted Living facility, will Medical ever be able to come after the house? I have heard that the house can be taken by Medical within 3 years of the sell/purchase. I purchased the home below market value for what she owed on it. She first had me signed onto the Deed as half owner and then I refinanced the home into my name only. I want to make sure that I won't lose my home one day due to her being on Medical and in a Nursing Home or Assisted Living home.

There is a possibility that this transaction might get challenged as fraudulent intra-family transfer. You and your grandmother need legal help ASAP.
