There are a series of forms that you and/or your parents would likely have to file in probate court. Each county generally has a website where forms can be found, and you should look under the section for probate, then guardianship, then seek out the appropriate forms. You'll need a petition and order for guardianship, a declaration, and several other forms, including a proof of service.
