A 4 year old 401k contribution being asked to be returned

My Brokerage firm has contacted me, on behalf of a past Employer, to state that there was an error made to my contribution cash out from 4 years ago and is requesting a portion of it to be returned. They are asking me to send a Money Order. Am I responsible for this error and is there a market calculation based on the current market losses that they should be abiding by as well?

If I do not return it, what then?
