If an employer documented an employee using an incorrect date as reference for the disciplinary action, does that nullify the documentation? If so, where is the law located to reference?

No. It may call into question the credibility of the document, if its accuracy is relevant to an issue in a legal proceeding. But an error in the document does not necessarily invalidate it.
