Health Care Premium Increase

My girlfriend is on disability from a work-related injury. She has been on disability for about a year. To continue her health and dental insurance she agreed to an increased payment of $100 a month. She just received a letter stating that she owed $1,500 ($300/month) for the past 5 months. While on disability, her employer increased her health/dental premiums without written notification. What legal recourse does she have?

More information is needed. She may have no recourse. If she is employed by an employer with 50 or more employees, she is entitled to 12 weeks of coverage under the same terms as when working That is, if she was required to pay a portion of the premium, she will continue to pay at the same rate. After the 12 weeks, she may continue her coverage but she must pay all of the required premium.

If the $1500 is the actual premium cost after the 12 weeks, it sounds like it may be correct, subject to an examination of the notices given to her by the employer.
