OK, seriously, I'm pretty sure it is a breach of professional ethics for us to advise you how to break Federal law, even if it legal under state law. Medical marijuana laws are administered primarily by the State Department of Health Services. That would be where to start. You should also probably read the texts of SB420 and Prop 215.

If you mean a 'medical marijuana card' for possessing small quantities "legally" in California, the 'growers cooperatives' that advertise in throw-away local magazines usually have some questionable 'doctor' issue and print you up one as part of your purchase and 'membership in the cooperative'. Keep in mind that possession is still a federal felony, with or without that cute little card. Growing is a BIG federal and state felony, and not subject to a 'license' in CA or anywhere else, except under FDA rules for prescription issuance by hospitals and hospices. Don't believe everything you hear in your local drug crash house. The OC DA, among others, is currently cracking down on such 'growers cooperatives' and arresting everyone involved. As they say: You pays your money and you takes your chances.
