Evictions on agreement violations

I manage a Mobile Home Park. One of our tenants has violated a law which is stated in there rental agreement as grounds for eviction. Is there a spacific court form to use for this eviction or is the standard unlawful detainer the forms to use? note, I have done my own evictions for years but have never had to deal with this type of situation.
