Legal Question in Real Estate Law in California

I owe a large uninsured hospital bill, and the hospital is asking me to give them a voluntary lien on my house (my primary residence). If I do so, can they then force the sale of my house to satisfy the lien?

There is no benefit to you in volunteering to have a lien put on your home. Do not do this voluntarily. Have the bill reviewed by an appropriate medical billing review company. Uninsured hospital bills are extremely inflated and in most cases an insurance company would be billed 20-33% of the amount you were billed. Consult a bankruptcy attorney. Maybe the entire bill could be wiped out and you could keep your home.

To answer your direct question, yes they would then be able to force a sale of your house.
