changing a trust

i would like to know what form i need to change the benifacary of a trust and delete proir heirs in trust with land in calif

this should be done by an attorney to ensure that it is effective. if it is an irrevocable trust, or one that has become irrevocable as a result of the death of a settlor or settlors, it cannot be amended (without court approval).

You can amend a revocable trust to change the beneficiaries of the trust, as provided in the trust agreement. I do not know of a place where you can obtain a form. I do prepare amendments for my clients.
