Legal Question in Workers Comp in California

I have a Worker's Comp injury, for which I was awarded lifetime medical care (just for that injury, of course). Although the case is in So. Cal., I have since moved to Wisconsin. The insurer does not respond to repeated phone calls by me, my doctor's ofc, or FAXes sent by my doctor's office (many since Jan., 2012). What form do I need to file to claim lack of providing standard of care? Can I file it from Wisc., and follow up, if necessary, when I'm in So. Cal. (I'll be there from May 23 through June 12)? I have had no attorney, and don't intend to hire one. Thank you.
