Add Name to Deed

What form(s) do I use to add my husband as an equal partner to the deed to our house. It is currently in my name only. I also need to change the deed into my Married Name. I believe I need a Quit form? Can I do all this at the same time?

You should sign a quit claim deed as the seller with the same name as on the existing deed, and transfer title into your married name and husband.
