Legal Question in Real Estate Law in Colorado

Grandmother died before signing over property

My Grandmother had a lawyer draw up papers for property in Colorado, City to be signed over to me. I never had a chance to get the papers signed before she past away in 1996. She also never signed the papers without me being present. Now my Father is the sole heir to everything and is willing to sign the property over to me because it was her wishes, but it is not stated in her Will. We don't know where to begin. Everything is still in my grandmothers name. I have the Deed and all papers needed for the property. I even retyped out the Papers she had drawn up and entered my Fathers Name and deleted my Grandmothers name. Where do I go from here? Can my Father and I just go to a Notary? Thank You, Aloha
