How to give my mother custody of my son legally

How do I legally give my mother custody of my 16 year old son while he is living with her attending school?

What does the father have to say about this? Do you want your mother to adopt your son or is he just visiting. If adopting then you must discuss this with the father. If he is just visiting then there is not much that you need unless you are expecting a problem from the father.
