What is the form called that you fill out to give someone permanent residency of a home and land until the time of their death?

It is not a simple form but you are talking about giving a life estate. A person who owns property may give or retain a life estate which is the right to live on the property for the term of a person's life.

There are tax issues, and other legal issues that should be examined before creating this property interest and the life estate can be created in the Deed to the property.
