Retirement Disability

I provided written statement from my doc stating I was unable to work due to anxiety and depression. My job kept saying they would send me forms for disability retirement. Never did. Then totally ignored me, refused my calls, and then terminated me, stating that I was physically unable to work. What recourse do I have, if any and within how much time?

Did you have sick days or PTO to use? What does your employment manual say? Did you have a disabiliy insurance policy either short term or long term? What did that say? These are just some of the questions to be answered. Feel free to call or e-mail me with any further questions.
