Adding names to a deed for a condo

My elderly mother would like to add my sister and I to her deed for a condo she owns in Delray Beach, Florida. She is a New York State resident. Besides filling out the Dr219 form, what other forms need to be filled out and where do I get them?

The only other thing is the deed itself, preferably a warranty deed rather than a quit-claim deed, so as not to terminate her title insurance. It would best be prepared by a Florida attorney, so as to assure a right of survivorship upon her death, which I assume is your desire. It would then be signed by her before two witnesses and a notary. If there is a mortgage on the condo, there would be documentary stamps due upon recording of $7.00 per thousand dollars of the outstanding mortgage, in addition to the $10 recording fee. Of course, each of your interests would then become subject to your creditors, in case you were sued for an automobile accident, or such.
