Can a person write up on a free form off the internet a power of attorney for everything , have it signed, witnessed and noterized, and does it need to be filed with any government office.

Yes, yes, and no (it doesn't need to be filed with any government office). In fact, there isn't even any government office that takes them.

Yes you can prepare a form from the internet. That said, it is a good idea to have it reviewed by an attorney. Of course, since you will have to spend money on an attorney to review it, you may as well have the attorney prepare it. I suspect you could find an attorney to prepare one for $75 to $100. That is an estimate and will vary according to your circumstances. And no it does not need to be filed.
