LLC Formation

I have a Internet Website business

a EIN number, A registered Business name. What else do I need to do in order to form a LLC?

If by a "registered business name" you mean you have registered the LLC name with the Secretary of State, you will also need to set up a business checking account and get an operating agreement to determine the ownership and the management of the company. (Similar to by-laws of a corporation.) Then you may need to maintain company minutes/consents depending on how the company is managed. Simply registering a business name is not sufficient to be a fully formed LLC.
