Minor Name change

what is considered abandonement in GA. My daughter's father and I were never married. there is an court order support order is that is current and there has been 4 visitations in 16.5 yrs. My daughter wants to hypenate her name to reflect both parents surnames to solidify her school records so she can get a DL & take SAT. The father will not respond the request

If he's paying support he hasn't abandoned the child. I'd have to know more facts to assess the possibilities of a name change now, and I do have a suggestion if it cannot be done now.
