Re: Agreement form for HealthClub

I am looking for an Agreement that states the customer will agree to a month to month, 3, 6, 12, membership and that dues will automatically be EFT out of their checking account. Also, if for any reason that the customer acts in a vile manner with sexual misconduct, stealing, vandilizing equipment something to that nature that if anyone does not apply to the terms and conditions of the Agreement that their memebership will be terminated. I am not good with the wording, but I need something along that line.

Hire an attorney to draft one for you. You are looking for custom language, so a form will do you no good.

If you are a member of a health club owners association there may be a form that organization recomends. If that is not the case you need to hire an attorney to draft an agreement for you.

You should also have signs constructed to post openly at your location concerning behavior and the consequences.
