Birth Certificate correction

Question is how do I get the last name on my birth cert. corrected to match the the name I have had for my entire life of 57 years. I recenty discovered that for some reason my mother had her maiden name on my birth cert. But raised me using my current name which is not the same.

DoI need to do a legal name change or what, and will I need an Attorney for this. I need direction & help on this.

You need to legally change your name. You can do it yourself, but it is a pain and you have to learn a fair amount about Cook County legal system. Our firm, for example, handles such simple matters on a flat fee basis. either way, its pretty amazing you went 57 years without using the name on your birth certificate!
