Legal Question in Civil Litigation in Illinois

I live in Illinois. A number of years after my mom got married (2009), I changed my last name to match her married name to avoid confusion. Now, she is divorced and I would like to change my last name back to my previous (and birth) name. Do I need to fill out the same form I filled out the first time (Decree and order of change of name)or is there an easier way?

You are right. You must follow the same procedure. This time the court may wonder why you're asking for a second change, but given the facts this should not be an issue as long as the purpose is not to avoid creditors...
