Dear Sir / Madam,

(Chandavar, Uttara Kannada, Karnataka)

Since you have the Birth Certificate dated 02nd October, 1989, you should make an application to the School providing them with a copy of your Birth Certificate requesting the School authorities to issue you a fresh Leaving Certificate with the correct date.

It is an uphill task but not impossible.
