Legal Question in Civil Rights Law in India

Is it compulsory to have the title name in ones Name?

Dear Sir/Madam,

My friend is a south Indian & his name is Mohanraj, here the name doesnt have any first, last or title name. When he try to obtain PAN (Permanent Account Number), he has been asked to mention the title name by authorities and they told that it is compulsory to mention the same.

As his name is appearing as Mohanraj only in all the statutory records what he has right now. So it would be mislead to add the title name here.

Kindly advice us, whether it is compulsory that a citizen should have a title name with his original name reference to our (Indian) constitution)?
