Legal Question in Credit and Debt Law in India

my case was dismissed in default by hon'ble consumer court. i did a mistake in hearing date.but now when i filed my restoration application to reopen the case, then court demanded one more application for condonation for delay.and i need a proper format for restoration application. I need the format of the application for condonation for delay. I will be very thankfull.

there is no specific format. just move an application stating the number of days of delay in filing the application for restoration.
