Legal Question in Credit and Debt Law in Indiana

Affidavit of Indebtedness

I have a hearing for Credit Card Debt, but can submit an Affidavit of Indebtedness prior to the court date, can you please tell me how to write one of these, & what I should include in it? Do I need a legal form or can I just write it on plain paper and have it notorized.

I never heard of an Affidavit of Indebtness as a pleading in a civil case. Could this be some device by a debt collector to trap you into an untrue admission? If you want to avoid a default judgment against you, you better attend the court proceeding yourself.
