Legal Question in Immigration Law in Indiana

Hello, I came to USA on nov 2007 on h4 which is valid till sep 30, 2009. I applied H1B on 2008 oct. I got approved with I94 till oct 2011. I never worked, my employer never paid. I had high risk pregnancy from 2010 jan. But he never explained anything about paystubs. Now i want to change my status from H1b to H4. How should i proceed.
