My direct supervisor was in my medical records I did have restrictions however a note was given to her everytime I went to the doctor. The proof that I have is a letter from my neuro to my family doctor we work in a medical office. She wrote on the note for me to ask the neuro to change what it says and signed her name. She also told me she could not give me a good evaluation because I have been out on FMLA. She stated on my eval out sick alot FMLA. I have since resigned due to her treatment she stated I was a burden because I took FMLA. Are there any violations in this post.

Your use of FMLA should not have been held against you and it would appear that your supervisor's comments may have restrained your use of leave. The fact that you resigned may work against you, but it does seem that you may have a cause of action against the employer. You should contact an attorney to discuss your situation in more detail.
