Petition to vacate streets and alleys

I am looking for a legal form to present to my Indiana town requesting to vacate streets and alleys in an old plat. I was told this needed to be submitted as a Petition for Vacation, but have been unable to find this form in the many websites I have researched. Any help will be appreciated.

Ask your town counsel if someone else has ever done it and then get a copy of their form. Otherwise, you can check the lawguru form database or go to IU's law school in Indy and see if their library has something.
